Description: Advance Practice Nurses possess expertise in the management of complex responses of individuals, families, and communities to actual or potential health problems. The APN role is multifaceted. APN's serve as direct care providers, consultants, researchers, and educators. The APN, as Practitioner, models excellence in practice through direct care to selected patients or groups. As Consultant, the APN does problem solving with individuals, families, group agencies, communities, and/or interdisciplinary colleagues (within and outside of the clinical setting) As Educator, the APN supports and participates in learning efforts of clients, staff and students within the clinical setting, area schools and institutions of higher learning. The APN, as Researcher, promotes scientific inquiry in clinical practice and uses research prsional Development by continuing to improve and augment personal skills and contribute to the health of society. The Nurse practictioner in the Cardiac Transplant Department provides patient management to both inpatients and outpatients, pre/post heart transplant or those patients with advanced cardiac disease. The Nurse Practitioner will also be responsible for the coordination of care between all disciplines on the team, participation in the work up and selection of Heart Transplant candidates. The Nurse Practitioner will participate in on call coverage for coordination of donor organ offers and subsequent transplantation.
